To complete the Issuer Sponsored Conversion form, follow the steps below:
Log in to your Bell Direct account and hover over My Account.
Select Forms, then choose the Issuer Sponsored Conversion Request form.
Complete the form using your Bell Direct account name and account number.
In the following section, enter the name and address exactly as shown on your dividend or share registry statement.
Provide your Bell Direct account HIN in the relevant field requesting HIN details.
Complete the table by entering details of the applicable securities.
Review the information provided, then physically sign with a wet-ink pen and date the form at the bottom.
